Your protected health information may be used and disclosed by the physician, our office staff and others outside of our office that are involved in your care and treatment.  Your protected health information may also be used to pay your health care bills and as part of the business operations of Center for Women’s Surgery.

Our Promise to You

Under the law, you as the patient have several rights that Center for Women’s Surgery is committed to upholding.  You may view our policies regarding your protected health information below.

Acknowledgement of Privacy Practices

Center for Women’s Surgery requires that you have a signed copy of the Acknowledgment of Privacy Practices statement in your medical records indicating that you have received a copy of our Notice of Privacy Practices.  You may download a copy to sign and bring with you to your first appointment.
